Sinus Infections:  Myth and Reality

There is probably no more confusing problem amongst common problems than sinus infections and sinusitis.

The confusion comes from the rapidly growing trend to call a cold a sinus infection to open the door to use of antibiotics.

Here is what is typically taking place across the nation.  A person gets a cold, a runny nose and cough follow.
After some number of days, the runny nose and/or cough become so annoying that medical help is sought to relieve the suffering.  So far everything makes sense.

But the next step is where the trouble begins.  Once at the doctors office, either the person or the doctor raises the question of a sinus infection.  Often, the doctor will say one is present and prescribe an antibiotic.

Now, it turns out that antibiotics kill bacteria very well, but if your infection is from a virus, they do no good, only harm.

So what is the story with the "sinus infection?"  What is it, a viral or bacterial infection?  For many, many years, the assumption has been that it is bacterial and antibiotics are needed to get rid of it.

But, finally, someone actually tested that idea.  They took 166 adults who met all the rigorous criteria for having a sinus infection.  A random half got 10 days of antibiotic, the other half got placebo.  And what they found was that the two groups did about the same.  Both missed the same number of days of work, both got better about the same rate, only a slight improvement advantage on day 7 was noted for the antibiotic group.

So, the study is clear, what we call sinus infections do not get better with antibiotics any faster than with sugar pills.  This makes it very likely that sinus infections are viral.

This actually makes a lot of sense.  Every cold, after all, is a sinus infection.  Viruses that cause colds never just infect the nose, they always also infect the spaces behind the nose, the sinuses. That's why when we have colds there is such a great, huge amount of mucus!

Now, it turns out that sometimes that mucus does get infected with bacteria and the mucus turns to pus.  As this study proves, that does not happen very often.  So how do you know when it does?

We would say if  your sinuses start getting very painful that may be a sign.  We do not mean the discomfort we all get with colds, but a shift to a very painful situation where tapping the face is very, very hurtful.

Just as importantly, there are several signs that many think indicate bacteria are now causing infection, but these signs do not mean you have a bacterial infection:

  • mucus turning from clear to yellow or green
  • having cold symptoms for a long time
  • fever
  • even white material filling the sinuses on CT or MRI scan- radiology images cannot distinguish between the mucus of a viral infection and pus of a bacterial infection.
This is why the study reported in the NY Times is so important.  It actually took nearly 200 people, each of whom met very stringent criteria for having sinus infection, each of whom had every reason to believe that their nasal congestion, runny nose, and cough indicated they had moved from a viral cold to a very bacterial infection, and none of them did.  Not a single person responded to antibiotic in a fashion seen in people with bacterial infections.   As noted the antibiotic did about as well as a sugar pill!

Bottom Line
So when you get a cold and have a runny nose, congestion, misery, cough, nose pain, you should know that almost certainly have an infection with a virus.  The very concept of a bacterial sinus infection is becoming suspect, and in most circumstances is simply an excuse to try a course of antibiotic, the famous "Z-Pak," for example.
Colds last a lot longer, create a lot more yucky mucus, and cause far more misery than we expect, or certainly more than we would like.
But we should respect the facts, and avoid use of drugs that we know will not work.
Key Point:  If you are infected with a virus, use of antibiotics can only harm, they do no good.

In September of 2010, Advanced Pediatrics cited a growing body of evidence that acetaminophen, the main ingredient in Tylenol, which is also found in many cold remedies, might actually cause asthma to occur in children.   http://advancedped.blogspot.com/2010/09/acetaminophen-serious-question-leads-to.html

Since that time there have been reports in The New York Times and now on February 20, 2012 on NPR, that it really is time to stop using Tylenol and any other remedy that contains acetaminophen.

The issue is that studies demonstrate a clear trend.  When we look at any group of children, the more acetaminophen they take, the higher the chance they will develop asthma.  Further, when a country adopts use of acetaminophen, that country's incidence of asthma goes up.

These are the two facts that led Advanced Pediatrics in 2010 to advise stopping use of acetaminophen.  Now we are hearing that study after study has found the same observations, time and again.  This has led a highly respected pulmonologist at Akron Children's, Dr. McBride to adopt the same line of advice Advanced Pediatrics offers:  Stop Using Tylenol and other drugs containing Acetaminophen.  This is the recommendation featured in the NPR story.

In the NPR story, some doctors correctly pointed out that we do not yet have proof that acetaminophen causes asthma to develop.  The data so far establish an association, children who take lots of acetaminophen develop lots more asthma.  But that may because those kids have other problems more than others, problems that really cause the asthma.  Further asthma, they point out, is likely caused by many factors.

But in our experience, whenever a drug, particularly an optional drug, is observed to cause a problem the more you take it, we find it prudent to stop using it until it is know to actually cause the problem or not.

BOTTOM LINE
We strongly recommend all families stop giving their children acetaminophen at this time.

Arsenic and Our Food

A investigator from Dartmouth College has done us all a favor and exposed the fact that certain foods we give our children have elevated levels of arsenic.


Two key conclusions can be drawn from his findings:
  1. The level of arsenic found in these foods was clearly over the permitted level.  The reported concentrations in the food were as high as 10-12 times the permitted level.
  2. The level of arsenic has not been shown to have caused any harm in any known individual.  This is a very important point.  Every substance has a toxicologic profile, meaning that there is an amount under which it is harmless and over which it begins to cause harm.  Obviously, if your exposure to a given substance is under the level that causes harm, the exposure will not be harmful at all.   We do not know if anyone exposed has had an amount of arsenic that will cause harm.
The Arsenic contaminated products

All of the products containing arsenic trace the arsenic to a brown rice syrup.   If a food you have contains no brown rice syrup, this warning is not for you.

Here is a list of those products we have ascertained contained the arsenic via the brown rice syrup.
YOU SHOULD DISCARD ALL SUCH PRODUCTS IN YOUR HOME NOW.
  1. Organic infant formula-  two brands of 17 tested had arsenic both had brown rice syrup as an ingredient.  One of those brands is Baby's Only Organic Formula
  2. Cereal Bars- again arsenic limited to those containing brown rice syrup
  3. Energy Shots- three brands tested, one had arsenic, and as before, contained brown rice syrup
Many wonder, how did a poison like arsenic end up in infant formula, especially an organic brand?  The answer is that all these foods were grown organically and so none had any arsenic applied to them.  But arsenic is present in the Southern rice fields from the days when those fields grew cotton.  Cotton is not eaten so nearly unlimited amounts of toxic pesticides can and are applied to those plants.  The arsenic applied in those times remains in the soil forever, and rice is a plant quite adept at sucking up arsenic in the soil.  And so, even on an organic farm, rice planted in arsenic containing soil will have arsenic in it.


Should you be worried?  No.

The next question now is just how much to be worried?  The answer is, not much.
The real impact of this finding is that we need to track the presence of toxic elements like arsenic in our food supply.  The fact that arsenic is present makes me think we should look for it and reduce its levels in our food.

The story mentions that arsenic is in some foods up to 12x the level set as safe.  Keep in mind that the level set as safe, as noted above, may be much lower than the level that will actually start causing symptoms.  We have actually heard from Dr. Brian Jackson, the lead investigator on this study from Dartmouth, who is an expert on arsenic.  A family in the practice has been in touch with him and passed these words from him along, "...you don't need to panic here, these are not acutely toxic concentrations."

No one has been reported with even one symptom or sign of any harm from arsenic exposure from these foods.  And, the scientist most concerned about arsenic states there is no reason for concern of any harm being done, as long as the food is no longer eaten.

Bottom Line
  • Scientists at Dartmouth have found arsenic in organic foods containing brown rice syrup.  These include 2 brands of infant formula (Baby's Organic Formula is one), cereal bars, and energy shots.
  • The amount in these foods exceeded safety thresholds, but was too little to cause any acute harm.
  • All foods with brown rice syrup should be eliminated from your home, but it appears no harm has been done to date.
  • This episode pushes me to want rules in place to keep arsenic out of our food and drink.

An Important Advance In Understanding Memory

This week The New England Journal of Medicine published an observation that dramatically illustrates that 
we live in the era when the mind may very well be figured out.


The observation had to do with memory.  Memory is one of those concepts that everyone knows about, but 
that we actually do not understand too well.

We all have learned facts that we can recall later in time, but just how does the brain take in that information, how does it get stored, and what allow us to retrieve the facts when we need them?

Researchers at UCLA and Tel Aviv University had the opportunity to stimulate a specific area of the brain and see what impact that had on the person's memory.  They were able to do this because one accepted treatment for very severe epilepsy is to thread a tiny wire deep into the brain to a spot where the seizures are generated, and then by running a very small electrical current, stop the seizures.

The researchers were able to place the wire at one spot in the brain of 7 adults who were going to have a wire placed for seizure control anyway.   The region of the brain is called the entorhinal cortex, which is the area around the yellow squares numbered 28 and 34 in this illustration from Wikipedia:

The entorhinal cortex is the part of the brain that connects the hippocampus to the pre-frontal cortex.  That's important because we know that new stories and facts are converted into memory in a structure called the hippocampus and then these newly minted memories are placed into various parts of the brain.  The pre-frontal cortex is the area in the front of the brain that has much to do with consciousness- being aware of thoughts, conscious decision making, feelings.

As you might imagine, anything that connects being able to form new memories to the ability to think and feel would be central to memory function, and that is the case with the entorhinal cortex.

What They Found
So when the electrical stimulation was on, the 7 adults were able to solve challenging visual brain-teasers more quickly and using shorter routes.  The researchers also saw changes in the adults' EEG that showed a difference in the flow of electrical information from the nerves of the brain.
Interestingly, if electricity was applied to the actual seat of new memory formation, the hippocampus, memory was not improved for this task.

What it Means
Two items jump out from this work.

The first is that the door is opening on a world in which we can change the underlying abilities of our mind.
In this instance, a current applied changed a person's ability to solve complex visual puzzles, a clear instance of changing a person's power of memory.
Other work has demonstrated that the mind can change its power of working memory, and phoneme recognition.
As time goes on, more and more functions of the mind will be open to improvement, offering hope to some of the most heartbreaking challenges people face- impaired thinking.

The second is that our understanding of just how the mind works really is growing.  This study illustrates that increasing the activity of nerves in one area of the brain leads to improvement in one type of memory.  Such information allows us to begin to really understand how memory really works.  What physically must happen for a fact or story to get recorded in our mind and recalled whenever we want it?

Of course, our hope is that as our knowledge of how the brain grows, not only will we be in a better position to help those whose thinking is impaired, but we will be able to help our children learn more easily and more effectively.  Schools will one day be places of expertise in neuroscience where teaching is designed to help children form memories that allow them to achieve wonderful levels of ability across a range of fields.

As with all realms of scientific progress, attention to making sure this knowledge is used appropriately will need to grow with the knowledge itself.

Good news!  The number of people infected with the influenza virus this year is very small compared to other years.

The influenza virus typically causes about 1/3 of all respiratory infections (e.g., colds, sinus infections, ear infections, pink eye, bronchitis, and pneumonia) during the winter.  Usually, it hits our region of the world around December through April.

In most years, during the winter, we see a big explosion of respiratory infections as the influenza arrives, but not this year.

Good data from the CDC support the observation that there is barely an influenza epidemic in the US or Ohio this year, with dramatically fewer cases seen.

Now, why are we so lucky?

There are two possible explanations, both could be true together:
  1. This just happens to be a light year.  Influenza infections are sometimes more common in one year than another, and this may just be a light year.  Personally I am skeptical of this explanation because the drop in number of cases is so dramatic, but time will tell.  If it is a random drop off, we will see the more typical epidemics in winters to come.
  2. The flu vaccine is working.  Clearly, if enough people in a community or nation get the flu immunzation, it could actually eliminate influenza infections, or at least, sharply drop their numbers.  We (the US) has been promoting influenza immunizations enough to suspect this is the cause.
One explanation that is very popular is one that we know is not true:  there is less flu because the winter is so mild.

Influenza infections occur only because of the influenza virus, and that virus sweeps across our region every December-April no matter the temperature, snow level, rain etc. We have seen very bad epidemics in warm winters and mild ones in cold winters.  So weather, temperature, and moisture levels likely play no role.

Bottom Line:  The winter of 2011-12 turns out to be a dramatically light one for influenza infections.  Flu vaccine likely helped make that happen.  Enjoy the lifting of the burdens of cold winters and of winter flus!

The Setup
That ultimate source of the shortages comes from the fact that the major choice of drug, by prescribing doctors, to treat ADHD in the United States are the stimulants, amphetamine (adderall, vyvanse) and it closely related cousin, methylphenidate (ritalin, focalin, methylin, metadate, daytrana).  And, as we all know, amphetamine is a major drug of abuse and illegal to use without a prescription.

This means that the US government is firmly committed to preventing, stamping out, and prosecuting illegal use of all the stimulants.   This is why you cannot get a prescription for them called in to a pharmacy, or have refills at the pharmacy available over a year.

One strategy the government employs is to count the number of prescriptions written by doctors, and then limit the number of pills manufactured in the US to that number.  It actually makes sense.   It turns out that in the US, doctors write 51.5 million prescriptions for amphetamine or methylphenidate a year, so drug companies should make about 51.5 million prescriptions worth of the drugs, and if they do, there should be no shortage.

That is what they do, so why shortages?

Money
Everyone knows that you pay more for a brand name drug than the same drug marketed as generic.  But did you know many companies make both versions of their drug.  The Times noted two such examples.  Novartis makes both brand name and generic Ritalin.  Shire makes both brand name and generic Adderall XR.

Now, if you made the same drug in a very expensive brand and a cheap brand, and could only sell one total amount of the drug, would you rather sell the expensive or cheap version of the same thing?  Turns out Novartis and Shire have opted for the more expensive.  This creates a shortage of the generic, a serious one.

Abuse.
Think about the situation from this angle now.  The US only makes enough drug to fill 51.5 million prescriptions.  If only 51.5 million prescriptions are filled, fine.  But what if 100 million are demanded?

That's what happens when so many law students and medical school students take these drugs illegally to boost their grades.  Or when a large number of college undergrads do the same.  Or when millions of kids and adults take these drugs to get high or stay awake.

Bottom Line
  1. There is a serious shortage of stimulant medication for ADHD.  The stimulants are amphetamine and methylphenidate.
  2. The shortage comes from three causes:
    • The drugs are under tight legal control to avoid abuse, so only a fixed number of doses are made a year
    • Drug companies have chosen to make more of their expensive brand and cut production of the same drug sold as a cheaper generic
    • Illegal use of the drugs is siphoning off supply
  3. We find the use of stimulants problematic from a health point of view.  In particular, the long-term effect on the human heart from  use of stimulants is not proven to be safe.  There is a medication that actually calms the mind, called Intuniv, that is showing promise as a drug to be used for ADHD without shortage, and likely with less harm.

A True and Astounding Scientific Breakthrough:  
Genes Changed, Christmas Disease Reversed


A profound triumph was reported in the New England Journal of Medicine this week.  On December 10, 2011, investigators in London made history.

For the first time in the history of the world, a virus was altered and then given to people, successfully changing the DNA of the infected person, thereby actually reversing a genetic disease, and ending the suffering caused by the genetic disease.

The report establishes that for the duration of the change in the infected person's DNA, their genetic disease is cured.

The disease is the second most common form of hemophilia, variously known as hemophilia B, Factor IX deficiency, or Christmas disease, after the name of a 10 year old boy with the disease, Stephen Christmas.  Stephen was the first person in whom the cause of hemophilia was defined as a deficiency of a clotting factor, specifically Factor IX.

Background:  The Basics of Why and the How Blood Clots
To understand the astounding nature of this breakthrough, a little background on hemophilia.  Hemophilias are diseases of too little clotting.  Blood is a very peculiar liquid.  It has to have two opposite properties available to it, at all times.  For most of the time, blood must be very liquid, it must be able to flow.  After all, it supplies all the oxygen and nutrient to the cells of the whole body.  No flow, no life.  

But at the same time, the delivery of blood depends on intact pipes to deliver it.  Leaks in the pipes lead to loss of blood, which can ultimately end flow.  Those leaks can be small or large.  All the pipes, or blood vessels, are made of living cells, whose edges always need to be kept nice and close to prevent any blood from leaking out.  The pipes can also be injured by tears and cuts.  So when tiny leaks occur or big tears, the blood right there needs to patch the hole if blood flow is to be saved.  And so blood has the incredible ability to convert from a liquid to a solid, right where the hole is, sealing the leak, in a process called clotting.

Now, for blood to always be liquid when necessary and at the same time be able to always be a solid when necessary, the body has devised many, many processes that keep the blood liquid, and allow it flash into being solid.  The fact that many, many processes are active in both directions, allows the body to fine tune the situation to a remarkable degree of accuracy, keeping the blood liquid and flowing whenever that is required, and converting it to a solid, in just the one spot it needs to be at just the one time it needs to be, whenever or wherever a leak occurs.

One of these complex systems of checks and balances is called the coagulation cascade, and it is made up of over 13 different enzymes, each called by their Roman numeral number.  So the #5 enzyme in this cascade is called Factor V.  In order for a clot to form, a sequence of one enzyme activating another needs to be launched before the final step of a clot forming can occur.  Of course, the body has various counter measures to stop the cascade from proceeding at every step, so that the jump towards making a clot can be very carefully controlled.

Hemophilia, then, is a disease where one of these 13 or so clotting factors is missing.  There are two clotting factors whose absence makes up nearly all of hemophilia, Factor VIII and Factor IX.   In Christmas disease, the gene that makes Factor IX is defective, and the affected person makes essentially no Factor IX.  This leads to life-threatening, painful, and damaging bleeding into bones and muscles. This is the disease that the descendants of Queen Victoria had.

If the person is given Factor IX, the bleeding does not occur and the disease is controlled.  But Factor IX is not easy to get, and to prevent bleeding one has to have a steady supply.  If one depends on getting Factor IX from donated blood, that exposes a person to lots of donors and a real risk of AIDS.  In fact, hemophiliacs were the people at highest risk for getting AIDS until the blood supply could reduce the amount of AIDS to essentially zero.   

Background on Gene Therapy
Now one more piece of background before we unveil the incredible nature of this news.

Genes are bits of DNA that make proteins.  And proteins, in turn, are the main signals of the body, directing it to do nearly each and every one of its myriad chemical reactions.  In the case of hemophilia,the gene for Factor IX makes the protein Factor IX, which we just learned is an important cog in the wheel that allows blood to clot at the right time and place.

No gene for Factor IX, no Factor IX, lots of painful and destructive bleeding.
So, if one could find a way to fix the gene for Factor IX in people with Hemophilia B, or Christmas disease, they would no longer have hemophilia.   To fix such a gene, one would have to succeed in each of these steps:
  1. Insert the normal gene for Factor IX into living cells of the person with hemophilia B.
  2. The normal gene would have to be active in the cells that normally make Factor IX.
  3. The cells with the inserted gene would have to actually make normal Factor IX in sufficient quantity 
  4. The new gene making the cells make Factor IX would have to continue operating the rest of the person's life.
Notice that in contrast to interventions like transplanting there is no risk of rejection or bad side effects.  At its heart gene therapy is the most harmless of ideas.  It simply restores normal function to your cells.  

But the great challenge of gene therapy has always been, how to get a new gene operating in your cell without disrupting that cell?   The answer these scientists came up with requires one more piece of background before we unveil the extraordinary breakthrough.

Background:  How does a Virus Work and how does that help make gene therapy happen? Therapy?
Most of us know viruses as a major nuisance and creator of agonies.  Viruses make us sick, they give us colds, flus, stomach flus.   They cause warts and rashes and cold sores.  Really, what good could come from that?  What sort of good science could possibly come out of a week of sneezing and coughing, and being up all night with your suffering child?

Well, it turns out that viruses are barely alive.  One could make the case that they are more little packets of information than of life.  Every virus that we know of is a very simple package of 2 elements.  A coating of a handful of proteins and within that coating a strand of DNA or RNA.   Each of these 2 elements have one specific task to complete.  

The coating of proteins contain codes that open the door of a cell the virus.  Keep in mind every cell in every bit of life must take in certain compounds, keep out certain compounds, and secrete certain compounds if it wants to live.  Cells are extremely clever about keeping what is needed in, what is dangerous out.  The proteins on the outside of every virus, then, are proteins that talk to their target cell to talk that cell into letting the whole virus into the cell.  Now, keep in mind that one virus is about one thousandth the size of one human cell.  So from a size point of view it is nothing for one virus to enter one cell, it just needs the right key, and that is what its coating proteins are, a key.

Once inside the cell, the second element of any virus comes into play.  That element is a strand of DNA or RNA.  Either way, that strand is really a piece of information.  And in the case of viruses, the information this strand of genetic material contains is limited to 2 items:  make more coating protein, make more viral DNA/RNA.  That's it.  That's the only message a virus delivers to the cell once it is inside, make more virus.
That information gets expressed by the viral DNA/RNA inserting itself into the cell's DNA thereby forcing your cell to stop what it is doing and make viral proteins and genes. 

Now, for most cells, that's a bad message.  For once the cell has been commanded to make more viruses, it makes billions of them and the cell pops.  That's how colds work, all the cells lining your nose, throat, lungs, sometimes eyes pop.  That leaves the lining of these areas actually burned, weeping and watery, red and swollen, and they stay that way until the cells are replaced and the virus is killed.  That is, a cold.

So why use such a marauding system to help someone get well?

It turns out there is no machine for delivering a new gene to your cell's genes like a virus.  Imagine, just plop the new gene into the protein coating of a virus, let the new gene sit next to the viral DNA/RNA, then let the virus into the patient's body, and whammo, the virus inserts all its genes into your genes and your cell now has the missing gene.

That's why science turned to viruses for gene therapy, they can deliver, implant, and activate the new gene!

But, to use a virus to fix your gene now demands these steps be met:
  1. Put the desired new gene inside the protein coat of  a virus.
  2. Once assembled, this new gene-virus combo still has to be able to enter the target cells.  The target cells are the cells of the patient's body that the new gene should help restore to the lost function.  In this case, the target cells are the cells that make Factor IX in the human body, and those turn out to be liver cells.  So, it does no good to have a virus with the gene for Factor IX strapped into it, if the virus cannot get into a human liver cell.
  3. Once the new gene-virus combo is launched in the body, it would be ideal to have the virus enter the target cell, insert the new gene into that cell's genes, but stop there.  That is, let the new gene in the virus be active in the target cell, but don't let the viruses own genes activate.  If the viral gene activates we know what will happen, billions of viruses will be made and the cell just cured of its genetic defect will pop, and the patient will just end up with a cold.
The Breakthrough

Now we are ready to be amazed.  The report from the New England Journal of Medicine establishes that this all has been done in people with hemophilia.

Scientists took a virus called the adeno-associated virus.  This is very unusual virus, in that it does invade and multiply in people, but does not cause illness.  A perfect sort of virus for gene therapy.

So what happened was that scientists altered the adeno-associated virus in two ways:  they inserted a normal healthy gene for Factor IX, and they weakened or deleted the ability of the virus to cause harm to the cell.

With this new machine in hand, they simply injected a bunch of this virus into 6 people with hemophilia B, or Christmas disease.   

Here is what happened:
  • Not a single person got sick
  • Every one of the six people with hemophilia needed less Factor IX injections to remain free of bleeding.  Four of the six went off Factor IX completely for the many months of the study.
  • The immune system did notice the virus and did make antibodies to it.
To put it in the simplest possible words, if this change sticks, 4 of 6 of these first people treated will be cured of their hemophilia!  And, the other two were nearly cured!

This is the first instance in the history of the world that a person had a normal gene inserted to take over from a faulty gene, curing a serious illness, with essentially no side effects.

The door is now very much open to addressing problems caused by single gene abnormalities.  Now, most of human suffering is not due to simple, one gene problems, but many problems are, like cystic fibrosis, muscular dystrophy, sickle cell anemia, for example.  And there may be conditions of more complex genetics where altering one or two genes could make a very large difference.

Think about, 6 people who did not make enough Factor IX to prevent serious bleeding now are making it on their own with no further intervention beyond one single injection of a very weak virus that did not make them ill.

I thought it quite poignant to report this astounding breakthrough to you on the Christmas of 2011.
After all, the first disease truly reversed with one gene therapy turns out to be Christmas disease.

The promise of medical science continues to amaze.  Modern surgery transformed our ability to remove harmful entities from our body, untwist clogged structures, repair wounded or malformed parts of the body.
Antibiotics gave us the unheard of power to kill deadly bacteria and push back the scourge of infectious disease.  And now, we can for the first time meet people who had one gene changed curing a once incurable disease.  We now have our hands on the genome, and the possibilities for helping life have suddenly taken another leap forward.  I am especially pleased that the approach to touching the genome was done so responsibly and safely.  As with every advance, a dark side always threatens, the dark side of unintended consequences.  But when I look at the interventions of surgery, antibiotic, and gene therapy, we may ultimately find gene therapy the safest of them all.  It's end point is clearly the most harmless, adding a normal gene to a normal cell, really simply restoring a cell to its intended and natural function.  No surgery or even antibiotic can claim that level of process, each involve killing normal cells (cutting) or disrupting normal functions (hurting helpful bacteria in the body) as they reach their goals.  If the delivery of the healthy gene can be made fully harmless, gene therapy could be the safest intervention yet invented.

So, I wish all families today a very Merry Christmas, a Happy חנוכה (Hannukah), and starting tomorrow Kwanzaa.   This breakthrough adds more hope that 2012 will be a Happy and Healthy New Year for all of us as well.
